Leaving Providence: Your 401(k)/Retirement Plan Options

Health systems merge, hospital names change, and retirement accounts quietly inherit the history. Leaving is the moment to find out exactly what you have and where.

Aduna Capital is not affiliated with, endorsed by, or sponsored by Providence. This page is educational, not advice, and describes general rules only. Plan details vary and change; verify with your plan administrator or Summary Plan Description before acting. General figures here were reviewed in October 2026.
The short answer

Money in a Providence retirement account can remain there, move to your next employer’s plan, roll to an IRA, or be distributed as taxable cash. Because the system has grown through affiliations over the years, add a preliminary step: confirm every account tied to your service, including any opened under a predecessor hospital’s name.

A system built by combination

Providence operates hospitals, medical groups and clinics across Los Angeles and Orange County, employing nurses, physicians, therapists, technologists and support staff throughout the communities around us. Like most large health systems, it reached its current shape partly through affiliations with other hospital organizations over the years.

The Form 5500 filings for 2024 (public at efast.dol.gov) list the 401(k) Savings Plan and Providence Health & Services 403(b) Value Plan (sponsor Providence Health & Services), each listing Fidelity as recordkeeper. Your statement, SPD or HR confirms any legacy plans.

Your four options

Once every account is located, each one runs through the same federal menu — independently, since the right answer can differ account to account.

Things to check in your specific Providence plan

Plan facts on this page come from the plan's public Form 5500 filing for 2024; plans change, so confirm with your current statement.

  • Accounts under legacy names. Your statement, SPD or HR confirms any legacy-plan balances.
  • Whether any plan is a church plan. The SPD states each plan's ERISA or church-plan status; confirm it before acting.
  • Fund accounts versus annuity contracts. Health-system 403(b) money sometimes sits in annuity wrappers with surrender terms — read the contract before scheduling a transfer.
  • The recordkeeper for each account. The Form 5500 filings for 2024 (public at efast.dol.gov) list Fidelity as recordkeeper for the 401(k) Savings Plan and Providence Health & Services 403(b) Value Plan (sponsor Providence Health & Services); confirm your current account on your statement.
  • Vesting, loans and after-tax amounts. Your SPD, statement or HR confirms vesting, loan and after-tax provisions.

Why the church-plan question is worth asking

Most private retirement plans answer to ERISA, the federal law that standardises disclosures, funding and protections. Plans sponsored by religiously affiliated organizations can elect church-plan status and step partly outside that framework. It changes paperwork, timelines and certain protections rather than your basic four options — but knowing which regime governs each of your accounts is exactly the sort of fact to establish before, not after, you file transfer forms.

Always transfer directly. Specify a direct rollover on every form. If a distribution is instead paid to you, IRC § 3405(c) mandates 20% federal withholding, and the balance only reaches its new home tax-free if you complete the rollover — withheld portion included, from your own funds — within 60 days.

Common questions

My hospital changed systems while I worked there. Where is my old plan?

Usually in one of three places: merged into the current system’s plan, frozen under the old plan’s name with a recordkeeper, or already force-transferred if small. HR and the current recordkeeper can trace it; your old statements speed the search enormously. Finding every dollar comes before deciding about any of them.

Does a 403(b) rollover work like a 401(k) rollover?

Substantively yes — same options, same withholding trap, same IRA and new-plan destinations. Differences show up in the plumbing: annuity contracts, vendor arrangements and, at some faith-based systems, church-plan procedures.

Will you tell me if my Providence plan is worth keeping?

Gladly, and it sometimes is — institutional pricing is real. Our fee does not depend on the money moving, which is the only arrangement under which that answer can be trusted.
